Tooth-Saving Procedures

Relatively recently, in any dentistry in Ukraine, a tooth that did not require filling was simply extracted. Modern dentists resort to extraction only in extreme cases. Dentists employ alternative methods aimed at tooth preservation – tooth-saving procedures. Such an approach is more justified, as even the highest quality prosthetics always fall short of natural teeth.

At the "Varshavsky" Dental Center in Vinohradar, experienced oral surgeons with profound knowledge of dystrophic and inflammatory processes in the oral cavity are working. During the operation, the specialist takes into account the peculiarities of the location of dental canals, vessels, sinuses, and other anatomical structures. The surgical procedure is painless and, provided that medical recommendations are followed during the recovery period, the risk of developing postoperative complications can be completely eliminated.

Indications for Surgical Treatment

Tooth-saving procedures are not a universal solution. Surgical intervention may not be suitable for all patients, and this approach is not always effective or advisable. In some cases, the dentists at the "Varshavsky" Dental Center still have to perform tooth extractions, dental implantations, or install removable or non-removable prosthetics. The primary indication for these measures is the ineffectiveness of conservative therapy. "Saving" a tooth through one of the tooth-saving procedures can be considered for the following diagnoses:

  • perforation of the tooth root;
  • inflammation of soft tissues and the formation of a cyst or granuloma around the tooth;
  • periapical inflammation of the tooth with a pin, post, or crown;
  • deep pocket at the gum-tooth junction, between teeth, and others.

The dentists at our "Varshavsky" Dental Center in Kyiv make decisions regarding specific tooth-saving procedures based on the case's characteristics, weighing the benefits of the procedure against potential risks. If the dentist has even a minimal chance of saving your tooth, they will definitely do so.

Types of Procedures

In the surgery of the "Varshavsky" Dental Center in Vinohradar, the following procedures are practiced within the framework of tooth-preserving therapy:

  1. Resection of the Tooth Root Apex. A tooth-saving operation involving the removal of affected bone tissue along with a portion of the root. The procedure is indicated in cases where conservative approaches are ineffective in the presence of inflammation, cyst or granuloma formation around the tooth.
  2. Hemi-section and Root Amputation. In this case, the dentist removes one or two roots of a multi-rooted tooth unit with partial amputation of its coronal portion. This method prevents further progression of pathological processes, fully preserves healthy tissues, and yields good results. The operation is most commonly performed on upper molars when conservative therapy is ineffective.
  3. Coronal-Radicular Separation. During this procedure, the surgeon divides (splits) the coronal part of a multi-rooted tooth unit. This approach is justified when the pathological focus is located at the point of divergence of the roots. As a result, the dentist manages to divide the tooth into two parts, onto which crowns are placed after appropriate therapy.
  4. Root-End Filling (Retrograde Filling). This procedure is performed when it is impossible to fill the tooth canal through the crown of the tooth. For example, when the canal is curved or contains a foreign object (pin). Retrograde filling allows the placement of a filling through a small opening in the bone tissue. Sometimes the operation is performed in conjunction with resection of the tooth root apex.
  5. Flap Procedures (Gingivoplasty, Osteocorrection, etc.). These methods are aimed at restoring periodontal tissues, preventing destruction of the connective apparatus and alveolar ridge.
  6. Curettage (Open and Closed). This involves the mechanical cleaning of formed cavities, gingival pockets. The procedure is typically one of the stages of comprehensive periodontal disease therapy.

In the "Varshavsky" Dental Center in Kiev, all types of tooth-preserving operations are performed using effective and safe anesthesia under the control of X-ray imaging.

Each clinical case is individual, so before performing surgical treatment, the dentist may recommend accompanying procedures such as retreatment of tooth canals, professional cleaning. The necessity of specific measures will be explained by the doctor during the preliminary consultation.

When Surgery is Not Advisable

Like any other surgical procedures, tooth-preserving operations have a number of contraindications. These include:

  • Blood disorders;
  • Oncological neoplasms of any location and stage;
  • Uncompensated immunodeficiencies;
  • Exacerbation of mental disorders;
  • Somatic pathologies in a decompensated stage (diabetes, cardiovascular diseases, etc.).

Our doctors pay special attention to anesthesia tolerance. The selection of anesthetic is made taking into account the patient's age, medical history, and individual body characteristics.
